    Description/Abstract
    The aim of this quality improvement (QI) project was to develop and utilize a sexual health assessment questionnaire based on the CDC’s Five P’s Model in order to increase the amount of sexual health risk assessments performed during wellness exam visits. Secondary outcomes were to increase screenings and STI awareness through patient education.
